Welcome to the web page for Christian Brothers University Sports Medicine. The following forms must be completed and on file with the Assistant Director of Athletics for Sports Medicine at the start of each academic year. Student-athletes are required to have at least $90,000 in college sports injury coverage by NCAA rules. Student-athletes must contact the Department of Athletics immediately if there is a material change or update to the information provided on these forms.
Student-athletes are expected to print, complete and send the information on these medical forms prior to arrival on campus to expedite medical clearance.
